The membership fee is used to website hosting and maintenance costs, printing costs and other administrative costs. Membership fees will only be used for the benefit of the MAHS group as a whole and a full account of the use of the fees will be provided at the request of any group member at any time.

The information provided in completing this form will not be released to anyone outside of the group. The information will be considered private and confidential and will only to be used for the purposes of group event planning and communication.
